Luteal Phase Support

Based around The British Clinic, a non-profit organization is created to assist “Mother and Father to be” in researching, planning and managing their entire “Baby adventure”……No question is regarded as too big or too small to be asked within the support group, “BABY ADVENTURE SUPPORT ENVIRONMENT (B.A.S.E.) “ . In this support group, the emotional side of this adventure is addressed, discussing truths and myths surrounding conception and pregnancy. The group meets regularly in Abu Dhabi,  to make contact, email newbase@live.com. There is no need to avoid handling babies of other people or crossing the road to the other side when seeing a woman with a pram!!! It is known that women who have been through “IT” before,  often offer the most support and empathy.

Luteal phase insufficiency is one of the reasons of implantation failure and hence the cause of assisted reproduction failure and miscarriages. Luteal phase defect is known to affect women with PCOS (polycystic ovaries), thyroid and Prolactin dysfunction. Low progesterone environment is induced by the medication used in assisted reproduction. In addition to the standard folic acid and vitamin supplements which are to be used prior to even the start of pregnancy enterprise, progesterone and other medication may be necessary to overcome this luteal phase insufficiency.

Progesterone

Progesterone and Progestins have been shown to increase the live birth rate significantly. Progesterone pessaries 400 mg to be used vaginally or rectally twice daily is the most widely used effectively.

Progestins : Dydrogesterone and 17 alpha-hydroxyprogesterone caproate have been used successfully to support the luteal phase.

Oestrogen

As the implantation process depends on the quality of the endometrium, it suggested that the use of  micronized oestrogen might help to improve the quality of the endometrium and hence make it more hospitable for embryo implantation.

Human Chorionic Gonadotrophin (HCG)

Human chorionic gonadotrophin indirectly support the corpus luteum by increasing the concentration of oestrogen and progesterone production. Administration of HCG  has also been shown to increase the placental proteins.  On this basis, it has been shown that its use significantly increase the pregnancy rate. However the  increased risk of OHSS makes its use rather restricted.

Prednisolone

It is believed that embryos might be exposed to bacteria and white cell “leukocyte” infiltration. It is therefore suggested on this basis that the use of  Prednisolone might help.

Vitamin C (Ascorbic acid)

Ascorbate depletion proved to lead to generation of reactive oxygen species which inhibits the action of LH and blocks steroidegenesis. Women with unexplained infertility have a lower antioxidant status in their peritoneal fluids. On this basis of an antioxidant action, Vitamin C (Ascorbic acid) is recommended to be used as luteal phase support.

Aspirin

Aspirin has been shown to increase the uterine blood flow. On this rational it has been used to improve implantation of the embryos in the uterine cavity.

Heparin

The therapeutic benefits of heparin are base on this compound’s ability to bind to aPL thereby improving trophoblast invasion and differentiation, resulting in successful implantation. This is in addition to its anticoagulant effect in later pregnancy, which reduces the risk of placental thrombosis and infarction.

Neupogen

Granulocyte Colony Stimulating Factor (G-CSF) is a natural cytokine (chemical messenger) which help to produce healthy embryos and healthy placenta. Neupogen (Filgrastim) is a synthetic (recombinant) form of G-CSF. This is now used as intrauterine implant to assist with maintaining the immune system in case of failed IVF cycles or recurrent miscarriages and shown to be successful. It is given as uterine wash 2 days before ovulation or egg collection and as subcutaneous injection every other day up to 12th week of gestation.

Endometrial Scratching

It is believed that endometrial scratching increases embryo implantation rate. This is thought to be achieved by two mechanisms : The first is by stimulating increase white blood cells which secretes so called growth factor which in turn controls embryo implantation. Secondly, endometrial scratching is believed to increase expression genes (switch of genes) thought to be responsible for preparation of the endometrium for implantation. The procedure is done around day 21 of the treatment cycle. Analgesia is given 1-2 hours before the procedure.

Complementary & Alternative Medicine

Pregnancy support would not be complete without few points about complementary and alternative medicine ….. In fact even the vocabulary used in this context has to be understood carefully. It is the ancient medicine which should be called “Traditional Medicine” not the other way around. It has a definite role to play despite the general lack of understanding about it.

Acupuncture, Acupressure, Chiropractice,  Reflexology, Osteopathy, etc.

All these have been used effectively for relieving discomfort of pains and aches as well as in difficulties of achieving pregnancy whilst still on the usual medical treatment.

Ante-natal classes is provided by the professional, the classes focus on breathing, relaxation and visualization as a preparation for childbirth

Nutritional therapy

Dietary adjustments are combined with nutritional supplements, plays an important role in reproductive health. Advice is provided at the British Clinic in addition to access to referral to specialist nutritionist and food intolerance therapist as needed.

Prayer and Spirituality

At times of difficulties, one needs to soothe the worried mind, strengthen the faith and renew the hope. This cannot be better observed than in the situation of the struggle with fertility and pregnancy and the fear of child birth. The power of  “healing” by prayers is linked with mind-body therapy and cannot be measured !! The connection to THE POWEFUL BEING gives hope and strength and definitely works.
